PRE-PARTICIPATION STUDENT-ATHLETE INSTRUCTIONS
DUE BY FRIDAY, AUGUST 1, 2025
Please READ ALL directions below in completing your pre-participation requirements
1. Go to Athlete Portal HERE
2. Enter your personal user/athlete ID and Password
- User/Athlete ID: This is usually your Last Name (if this does not work - please contact your athletic trainer (email below) to obtain your user/athlete ID)
- Password:
- For first time users and those who have forgotten their password, please select "Forgot your Password?"
- Use Pitt email address to reset password.
- Database: atsupmc
3. Go to ATHLETE INFORMATION ICON
GENERAL TAB
- Please add and/or update ALL highlighted sections with your personal information. Please also enter your SS#.
- The 1st Address is your Home/Permanent Address. The 2nd Additional Address is your school address.
- Make sure the "Year" is the correct year you plan to graduate from UPG.
- Returning Athletes: Please add to Alerts/Allergies/Medications. DO NOT remove anything from your list except medications you are no longer taking.
-Click "Save or Verify Athlete Information".
SICKLE CELL TAB (Sickle Cell Testing is required for NEW Athletes ONLY)
The NCAA requires you to provide Sickle Cell Solubility Test Results
- Some states use this screening test at birth. You may attempt to obtain your test results from your pediatrician, PCP or birth hospital. It must be a Sickle Cell Solubility Test.
- If you are unable to obtain your prior results, you'll need to complete one of the following:
- Coordinate Sickle Cell Solubility Testing with your PCP
- THIS WEBSITE allows student athletes to order, pay for and schedule their Sickle Cell Solubility Test through Quest and obtain test results without seeing a physician. (MUST BE 18 OR OLDER TO USE QUEST)
Under the Sickle Cell Tab
- Enter your Sickle Cell Test Result (Negative, Positive SC or Positive SCT)
- You MUST Upload a legible image of your Sickle Cell Solubility Test Results
- Click “Save Sickle Cell Information”.
SCREEN/TEST/VACC Tab
- Nothing to complete
PAPERWORK Tab
- Nothing to complete
INSURANCE Tab
- You are required to have personal medical insurance in order to participate in athletics.
- Add/Update all of your CURRENT/VALID Primary Medical Insurance information (including Policy Holder information - this is usually one of your parents - the one whose insurance you are under).
- Upload separate FRONT and BACK pictures of your CURRENT/VALID Medical Insurance Card (Most insurances change on Jan 1st or July 1st so be sure you have the most up to date card uploaded)
(Recommend using cell phone to upload picture).
- If student-athlete has secondary insurance, please add that insurance information (Payor #2).
- Contact your medical insurance company and MAKE SURE your insurance will cover injury/illness while in Pennsylvania & specifically within the Pittsburgh area. Many out-of-state insurances do not cover you in PA.
CONTACTS Tab
- Enter a minimum of TWO Emergency Contacts
- Do NOT enter a login or password
FORMS Tab
- Click on the "Form Name" drop-down box on bottom left.
- Select Form #1, click "New", complete form, click "Save".
- Repeat this process for Forms #2-7.
- Be sure to "SAVE" each form before moving to the next form.
- All Student-Athletes must complete ALL 7 forms EVERY Year.
E-FILES
- Select Upload Document
- Enter Description as: 2025-26 Physical Packet (note Page 1, 2, 3, and 4 if uploading separately)
- Scan and upload all four pages of your 2025-26 UPG Athletics Physical Examination Packet.
- Select "Upload".
4. 2025-26 UPG ATHLETICS PHYSICAL EXAM PACKET
- You can download the 2025-26 UPG Athletics Physical Examination Packet by clicking HERE
- Please complete all of pages 1 and 2 and the top of pages 3 and 4 prior to your exam.
- Take all four pages to your physical appointment. The doctor will need to review and sign off on pages 1 & 2 and complete and sign your physical.
- You MUST use the designated 2025-2026 UPG Athletics Physical Examination Packet.
- Your physical CANNOT be dated before May 1, 2025.
-After your physical has been completed, please scan and upload all four pages directly into the Athlete Portal "eFiles" tab as instructed above.
5. ADD/ADHD PAPERWORK:
- If you are taking ADD/ADHD medications, please contact Karen Andreycak or Amanda Trayers (emails below) for more information on the required documentation.
- Your physician will need to complete and sign paperwork and provide pertinent information regarding your diagnosis and prescription on a yearly basis.
- Please address this EARLY as it often takes a long time for your physicians to gather all necessary documentation.
